Context
The Home Patient Telemonitoring System developed by SATEC for the Public Health System of La Rioja aims to address the growing demand for care for complex chronic patients requiring home transfusions.
With increased life expectancy and the need to perform transfusions outside the hospital environment, ensuring patient safety and care quality becomes crucial. This project seeks to integrate advanced telemedicine solutions to improve patient management and monitoring, ensuring procedures are carried out with maximum safety and efficiency.

The project faced several significant challenges:
Ensuring the Safety and Effectiveness of Home Blood Transfusions
The need to control and monitor home transfusions presents additional challenges compared to hospital-based transfusions, especially in terms of safety and real-time monitoring.
Integration with Existing Hospital Systems
The platform needed to seamlessly integrate with the Hospital Information Systems of Hospital San Pedro in Logroño (Selene), requiring compliance with specific standards like HL7 to ensure efficient and accurate data flow.
Implementing a Simple Authentication and Access System
The solution needed to integrate with Rioja Salud's Active Directory and CAS Authentication Server to enable Single Sign-On access (SSO) from the HIS without requiring multiple user credentials.
Developing an Effective Notification System
An SMS notification system needed to be integrated to alert about critical clinical or technical events, ensuring swift communication with responsible parties.
Solution
SATEC developed a centralized Telemonitoring platform that effectively addresses these challenges. The platform can receive, manage, and display all relevant information about patients in Home Hospitalization, integrating fully
with the Hospital Information System of Hospital San Pedro in Logroño. This integration is based on the HL7 standard, ensuring smooth communication between systems, from patient registration to result delivery.
The platform was also integrated with Rioja Salud's Authentication Services, allowing simplified access through Single Sign-On from the HIS, which improves system access efficiency and security. This integration reduces the need for multiple credentials and simplifies access management for users.
Additionally, the platform generates detailed reports attached to patients' Clinical Records, facilitating comprehensive case follow-up.

Improved Safety in Home Blood Transfusions
The ability to monitor patients in real-time and manage alerts has increased the safety and efficiency of home-based blood transfusions.
Successful Integration with Hospital and Authentication Systems
The platform has been effectively integrated with the HIS of Hospital San Pedro and Rioja Salud's authentication services, enabling a continuous data flow and simplified user access.

The integration of an SMS notification system has enabled a rapid response to critical events, improving the ability to address clinical or technical issues effectively.
